ABOUT HEIDY
Heidy is a certified nurse Care Manager who is PRI and screen certified. Heidy has a Bachelor of Science degree in Nursing from Dominican College in New York and has been a Registered Nurse for over 20 years. She has a broad range of nursing experience in both acute and home care settings and has specialized expertise in community health and care coordination. Heidy has managed and coordinated complex health care needs and services for clients with acute and chronic illnesses for 15 years.
In her role as a clinical nurse coordinator, Heidy has expansive experience in the role of health advocate and liaison across the care continuum among clients, families, medical providers and facilities. She has worked as a concierge, as a private duty nurse caring for high profile clients, and has delivered the highest level of professional nursing throughout her entire career.
Heidy’s is compassionate, thoughtful, and tailors her work to each aging adult and their network of family and caregivers. The goal is to provide the most positive, life-affirming, and tailored plan to ensure comfort and safety, with both empathy and compassion. As a certified Care Manager, Heidy works in conjunction with family attorneys by performing extensive assessments and preparing full reports for guardianship cases.

ABOUT KELLY
Kelly obtained her Bachelor of Science degree from Plymouth University and her Master of Social Work degree with a concentration in Gerontology from Fordham University. She has both Administrative and Director experience and has worked in a number of different health care settings. This includes skilled nursing, rehabilitation facilities, independent and assisted living facilities, hospice and hospital settings. She believes in working with families in a collaborative process to evaluate, plan, implement, coordinate, and monitor the options and services that best meet their needs.
She has strong experience in all aspects of care from admissions, discharge planning, and the assurance to provide follow-up community services upon discharge. She builds strong and effective relationships between families, administrative staff, and community and social services professionals.
Her passion and dedication shine through in her advocacy for patients, families, and caregivers. She sees her role not only as a practitioner but as a supportive guide, working to navigate individuals through their health and wellness journeys. Her commitment extends beyond the immediate treatment to encompass a comprehensive approach that addresses the emotional, social, and practical aspects of healthcare.

MATTHEW LOUIE, RN
ABOUT MATTHEW
Matthew is an RN with a Bachelor's degree in Nursing from NJ, graduating with honors, with just under a decade of experience in the field. His work has ranged from hospitals to nursing homes to homecare and beyond. Broadly, he has worked in the field of geriatrics for the entirety of his career managing complex medical cases both acute and chronic. He is adept at navigating everything from routine medical matters to hospital admissions, inpatient treatments, discharge planning and, most importantly, hospitalization avoidance. Specifically, he will work with you to create a comprehensive plan of care to best meet all of your needs: physical, medical, emotional, social and more. A unique plan tailored to your unique and incredible life.
