WHAT IS A CARE MANAGER?

A Care Manager is a professional who specializes in assisting adults and their families with long-term solutions and care arrangements.  The goal is to provide solutions that prioritize both the senior’s and the family’s goals, values, and wishes.

 

WHAT DOES A CARE MANAGER DO?

Through assessment of the individual’s needs and ability to function safely and independently in his/her own home, I evaluate the individual’s physical, mental, financial, and psychological well-being.

 

WHAT IF WE NEED LONGER TERM SUPPORT?

Heidy will be with you every step of the way.  She monitors the individual to ensure continuous quality care in all types of settings – whether at home or in a residential or skilled care setting.  She visits on a regularly scheduled basis to evaluate the client’s well-being, and to respond to any changes that may occur.  If requested, she can keep the extended family fully informed and recommend necessary or desirable adjustments to the care plan.

 

CAN WE AFFORD A CARE MANAGER?

Initial consultation, assessment and follow-up are billed on an hourly basis.  All fees for professional care management services are stated in the fee agreement.  This agreement must be signed before services commence.  Each management service is uniquely tailored to your family, your needs, and your budget.

 

WHAT DO WE GAIN FROM YOUR EXPERTISE?

Clients receive the personalized care of a private care manager supported by a network of top agencies and service providers for the elderly.

Heidy will begin with an in-depth assessment at your home or other convenient location to get a clear picture of your situation.  As needed, she will speak to relatives as well as other involved professionals to develop the most viable plan in partnership with clients and their families.

 

Each client’s needs, as well as the needs of those closest to them, are given serious consideration.  She pays special attention to the unique physical, social, psychological, and financial circumstances of each client. She is on top of current medical, government, legal, financial, entitlement and other programs where clients might be eligible for assistance. Her interviews and research give her the ability to provide you with a plan specifically designed for you and your family.

 

WHEN DOES IT MAKE SENSE TO ENGAGE A CARE MANAGER?

Every situation is different, but some signs that care management may be right for your family or loved ones include the following:

  • You are trying to care for an aging parent or adult from a long distance

  • Your aging parent is struggling with taking medications, getting to doctor appointments or following their doctor’s advice

  • Your loved one has a mental status change or complex medical issues that need coordination

  • An aging parent or family member needing assistance with home care, nursing home or assistive care facility placement

  • An aging parent is struggling with grief, bereavement or depression following loss of a loved one, partner or spouse
